Wat is Vitiligo

Vitiligo is een aandoening waarbij de huid en het haar hun pigment verliezen. De pigmentcellen worden vernietigd en de stof die de huidskleur bepaalt, melaninen, wordt niet meer aangemaakt. Hierdoor verschijnen witte plekken op de huid, vaak omringd door donkerder vlekken. Er is dus sprake van zowel hypopigmentatie (te weinig kleurstofpigment) als hyperpigmentatie (teveel kleurstofpigment). De afbeelding laat zien hoe Vitiligo er uit kan zien.

Vitiligo is an autoimmune disease in which pigment cells (melanocytes) are destroyed, resulting in irregularly shaped white patches on the skin. The picture shows how Vitiligo looks. Any part of the body may be affected. Common sites are exposed areas (face, neck, eyes, nostrils, nipples, navel, genitalia), body folds (armpits, groin), sites of injury (cuts, scrapes, burns) and around pigmented moles halo naevi). The hair may also go grey early on the scalp, eyebrows, eyelashes and body. White hair is called ‘poliosis’. The retina may also be affected.

Was ist Vitiligo?

Bei der Vitiligo(Weissfleckenkrankheit)handelt es sich um eine Erkrankung, bei der die Betroffenen im Laufe ihres Lebens charakteristische weiße, scharf begrenzt Flecken entwickeln. Größe, Form und Anzahl können unterschiedlich sein. Vitiligo kann in jedem Alter auftreten. Untersuchungen haben gezeigt, daß bei Rund der Hälfte der Vitiligo-Erkrankten der Pigmentverlust im Alter von 10 bis 30 Lebensjahren beginnt und dann mit unterschiedlicher Geschwindigkeit fortschreitet. Diese Krankheit ist nicht ansteckend.

De oorzaken van Vitiligo zijn helaas nog steeds niet bekend. Er zijn wel diverse theorieen die de oorzaak proberen te verklaren.

  De Gentse Universiteit heeft een nieuwe gel ontwikkeld waarmee de huidziekte vitiligo in bepaalde gevallen succesvol kan behandeld worden.De gel wordt gemaakt van "goede", gepigmenteerde huidcellen van de patiënt. Er wordt wat huid weggesneden ter hoogte van het zitvlak waaruit huidcellen worden geïsoleerd die dan in een gel op de aangetaste huidvlek wordt aangebracht. Drie weken later krijgt de patiënt UV-stimulatie, waarop de vlekken opnieuw pigmenteren. De behandeling duurt meestal een tweetal maanden.

PUVA is een behandeling met het medicijn Psoraleen en ultraviolet licht (UVA). Via deze behandeling kan er herpigmentatie (bruine puntjes) optreden in de witte plekken.

Psoralenen

• Oxsoralen (Methoxsalen) lotion
  Methoxsalen is a naturally occurring substance found in the seeds of the Ammi majus (Umbelliferae) plant and in the roots of Heracleum Candicans. It belongs to a group of compounds known as psoralens or furocou-marins. Is used as a topical repigmenting agent in vitiligo in conjunction with controlled doses of ultraviolet A (320-400 nm) or sunlight.

UVB (narrowband UVB), TL01

Ultraviolet B is, net als ultaviolet A, een bestanddeel van het natuurlijk zonlicht. Het heeft een krachtiger werking dan UVA en wordt daarom zonder aanvullende orale medicijnen, zoals psoraleen, toegepast. Sinds enige jaren zijn er nieuwe UVB lampen in gebruik die bijna even effectief zijn als PUVA therapie.

Huidtransplantatie

Kleine stukjes normale huid van de patient zelf kunnen worden getransplanteerd naar vitiligoplekken. Meestal worden kleine 'eilandjes' gezonde huid in de vitiligoplekken gelegd. De bedoeling is dan dat de pigmentcellen uit de gezonde huid in de vitiligohuid kruipen. Deze therapie wordt gecombineerd met lichttherapie. Voorwaarde voor de therapie is dat de vitiligo zich al enige tijd niet uitbreidt.

• Onderzoek naar transplantatie als genezing voor vitiligo
  Onderzoekers van het Universitair ziekenhuis in Gent, onderzochten de effectiviteit van het toepassen van transplantatie bij 28 patienten om vitiligo te genezen.

Bleking

Deze methode wordt ook wel de omgekeerde methode genoemd. Deze behandeling houdt in dat de gezonde huid afgebleekt wordt zodat de huid een egale kleur krijgt. Dit zou cosmetisch een mooier zicht zijn. Het afbleken gebeurt met behulp van een hydrochinon-bevattende crème. Dit is ook de behandeling waar Michael Jackson voor gekozen heeft.

Calcipotriol (expirimenteel)

Dit is een afgeleide van vitamine D3 toegepast in een creme. Het is een product dat wordt gebruikt voor psoriasis, maar het zou ook een gunstig effect hebben op vitiligo. Zowel calcipotriol alleen, als in combinatie met lichttherapie zouden een goed effect op vitiligo kunnen hebben.

Laser (expirimenteel)

• Excimer laser therapy
  In a recent study, vitiligo patients, most of whom had failed conventional therapies, were treated with the excimer laser three times a week. Fifty-seven percent of the vitiligo patches treated in the study experienced partial to complete repigmentation in only two weeks. The Xtrac excimer laser represents one of the newest great advances in laser dermatology. This laser emits narrow band ultraviolet B light and has been shown to promote re-pigmentation in those who have lost pigment in their skin.

  Een pigmentstoornis veroorzaken om kwaadaardige huidkanker (melanoom) tegen te gaan; het zou weleens kunnen werken. Daarom gaat de onderzoeksgroep van immunologe Rosalie Luiten met Vidi-geld na hoe de witte-vlekkenziekte vitiligo ontstaat. Als ze achterhalen waarom afweercellen de gezonde pigmentcellen vernietigen ligt de weg niet alleen open naar een behandeling voor vitiligo maar ook naar een remedie voor melanoom, waarbij pigmentcellen gaan woekeren.
